About the Role :
As a Primary Care Physician (PCP) with ChenMed, you will play a crucial role in delivering exceptional care to our patients. Your responsibilities will include :
- Patient Care : Conduct comprehensive assessments, physical exams, and develop personalized care plans. Manage both acute and non-acute conditions and provide clear documentation.
- Collaboration : Work independently and as part of a multidisciplinary team. Participate in clinical rounds, meetings, and quality management programs.
- Advocacy & Education : Act as a patient advocate, facilitating family conferences, and ensuring education on care plans.
