Position Summary
The Certified Assessor completes comprehensive assessments and planning services for people who are requesting long-term services and supports.
Essential Functions / Responsibilities

Position Duties
Demonstrate best practices in assessment and support planning, using a person-centered approach, providing and explaining information, facilitating access to services, and following-up to assure people get connected.Assure people have information about needed health care programs by sharing information about medical assistance and other health care programs, facilitating State Medical Review Team disability certification (if indicated), and informing people about long term and home and community-based services and supports, programs and service providers.Determine eligibility for home and community-based programs and long-term services, whether state plan services can meet the person’s assessed needs and supports, the person’s level of care, case mix and home care ratings, provide guidance to people in selecting services and recommend referrals to other professionals and agencies as needed for cost-effective long-term services and supports.Complete a Community Support Plan with each assessment and, for individuals who will receive state plan PCA / CFSS and Private Duty Nursing services, a Coordinated Services and Supports Plan, with a letter that includes your recommendations for the individual and case manager.Collaborate as needed to provide an effective transition of the assessment to a case manager.Coordinate with others to assure transition assistance to individuals residing in a nursing facility, hospital, regional treatment center or licensed supervised living facility who request or are referred for assistance.Manage any appeals to level of care and eligibility determinations related to assessments you’ve completed.Participate in the county’s multidisciplinary long-term care consultation / certified assessor team.Authorize and enter PCA services and rates in the Medicaid Management Information System (MMIS).Maintain up to date information regarding DHS regulations, rules, and procedures.Non-Essential Functions

Education and Experience
Bachelor’s degree in social work or related field and at least one year of home and community-based experience.Or Nursing Degree with certificate in public health nursing or closely related field and at least one year of home and community-based experience.Or current license as a registered nurse and at least two years of home and community-based experience.Or an equivalent combination of education and experience sufficient to perform the essential functions of the job.Required Training
