Pride Health is hiring Physical Therapist → Home Health to support our client’s medical facility based in Newton Center, Massachusetts. This contract opportunity is a great way to start working with a top-tier healthcare organization!

Job Summary :

The Home Care Physical Therapist is responsible for delivering comprehensive physical therapy services to patients in their homes. Patients may range in age from newborns to older adults over 100 years old. The PT evaluates physical impairments, functional limitations, and disabilities resulting from injury, disease, or other causes, and develops individualized treatment plans to restore function and improve quality of life.

This role requires close collaboration with the patient, family members, physicians, and other allied health professionals to assess needs and coordinate appropriate services. The Physical Therapist is expected to maintain thorough and timely electronic documentation and participate in effective interdisciplinary communication in accordance with agency policies, procedures, and MGH Home Care standards of practice.

Requirements :

Active Physical Therapist (PT) license in the Commonwealth of Massachusetts

Minimum 1 year of home care experience

Ability to travel to assigned patient cases

Weekend rotation required once every 6 weeks

Pay Range : $2800 / Weekly

• Offered pay rate will be based on education, experience, and healthcare credentials.
