What You'll Do :

Deliver skilled, compassionate in-home therapy as part of a collaborative care team

Evaluate and develop individualized treatment plans for adult and geriatric patients

Serve as a key liaison between patients, families, and interdisciplinary team members

Document care clearly and thoroughly in our EMR system

Help shape the future of in-home rehabilitation

What You Bring :

Doctor of Physical Therapy (DPT) or equivalent (MPT, MScPT)

Current and unrestricted Virginia Physical Therapy license

Two or more years of experience in home health, acute care, or rehab preferred

Excellent communication and critical thinking skills

Valid driver's license, active auto insurance, and CPR / BLS certification
