Must-Have :

Previous discharge planning and acute care case management experience

Experience in progression of care

Experience meeting with families and discussing plans of care

Description :

Case Managers provide care for patients in all acute care departments.

The RN Case Manager will most likely remain in the same area during their contract.

Typically, travelers are not required to float to multiple areas during their contract.

Patient ratios are 1 Case Manager to 16-20 patients.

Support on the unit includes Case Management Assistants, Lead Case Managers, Social Workers, and Utilization Review RNs.

Charting system utilized is EPIC.

Scheduling includes a weekend rotation of 2 weekends per 13-week assignment.

Typical staffing support during weekend shifts includes 3 RN Case Managers, 1 Social Worker, 1 Lead Case Manager, 1 Case Manager Assistant, and 1 Utilization Review Nurse.

There is no on-call requirement.

Holiday expectations include 2 out of 4 major holidays.

Approval of time off is up to 7 days and must be extended for any full week off.

Scrub color is navy blue or professional attire.
