Family Nurse Practictioner (FNP)

Rising Health Solutions is a patient-centered wellness and integrative care practice specializing in weight management, IV nutrition therapy, vitamin optimization, and innovative mental health treatments. We are expanding our clinical team and seeking a dedicated Family Nurse Practitioner (FNP) to join us on a part-time, as-needed basis with the intent to increase weekly hours over time.

Job Summary

The Family Nurse Practitioner will provide high-quality clinical care to clients seeking weight management, nutritional support, and adjunct wellness services. This position begins with 1–2 days per week as needed, with planned growth to 2–3 days per week , and includes provider coverage when needed .

Responsibilities :

Perform comprehensive assessments, diagnoses, and develop treatment plans for weight management clients.

Provide and oversee IV nutrition therapy and Vitamin injections.

Collaborate with the clinical team to ensure safe and effective patient care.

Maintain accurate and thorough clinical documentation in accordance with regulatory and clinic standards.

Participate in training to competently cover ketamine infusion therapy clients as needed.

Support clinical operations by providing provider coverage during vacations, sick days, or peak patient times.

Current and unrestricted Family Nurse Practitioner (FNP) license in the state of Arizona.

Active DEA license.

Experience in weight management, wellness medicine, or integrative / functional preferred but not required–training provided.

IV therapy experience strongly preferred.

Strong clinical assessment skills and commitment to patient-centered care.

Comfortable working independently and as part of a collaborative team.
