What You'll Do :

• Provide compassionate patient care by triaging phone calls, completing patient care visits, attending death visits, and ordering supplies and equipment.
• Lead and supervise weekend care teams, ensuring smooth operations and continuity of care.
• Conduct hospice assessments , symptom management visits, and support patient admissions.
• Triage patient and family phone calls , providing timely clinical judgment and guidance.
• Support and educate families , caregivers, and patients during critical end-of-life transitions.
• Collaborate with interdisciplinary teams to maintain consistent, compassionate care.
• Mentor nursing staff , promote team cohesion, and uphold care standards during Weekend hours.
