Care Coordinator MSW

Hourly Pay Range : $30.46 - $45.69 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors. Position Highlights : Position : Social Worker, Continuum of Care Location : Highland Park Hospital Full Time / Part Time : 40 hours / 4-10 hr. shifts 8 : 30a-6 : 30p Hours : Saturday, Sunday, Monday with an additional weekday shift, with 1 major winter and summer holiday coverage What you will need : License : LSW or LCSW license required Education : Master's Degree from an accredited school of social work required Certification : CPR BLS Certification required Experience : Two years' post-graduate experience preferred; healthcare and / or mental health experience preferred Skills : Experience with Electronic Medical Record (EMR) platform preferred. Experience with MS Office.
