As a pharmacy technician under the supervision of the Pharmacy Manager, you will play a crucial role in supporting medication adherence through telephonic outreach to our members and providers. You will assist in coordinating member care and facilitate referrals to our pharmacists and nursing teams.
Key Responsibilities :
- Engage with providers and members to identify patients at risk for medication non-adherence and conduct targeted telephonic outreach.
- Identify and address barriers to medication adherence in collaboration with members and providers, aiming to enhance adherence rates.
- Coordinate communication among various teams to resolve medication-related issues, including formulary changes and prior authorizations.
- Monitor and report on provider-specific adherence metrics and identify opportunities for improvement.
- Gather and coordinate member information to support discussions with pharmacists.
- Assist in coordinating medication therapy management (MTM) and transitions of care initiatives alongside providers.
- Refer members to nurse health coaches for enhanced education, care coordination, and comprehensive medical management.
- Provide valuable member education on pharmacy vendor solutions and available resources.
- Ensure precise documentation of provider interactions and interventions is maintained.
Qualifications :
High school diploma or equivalent; associate degree or higher is preferred.Certified Pharmacy Technician (CPhT) credential is required.Minimum of 2 years of experience in a pharmacy or managed care environment; experience dealing with providers is a plus.Exemplary communication and relationship-building skills are essential.Knowledge of pharmacy benefit systems, claims data, and quality metrics is preferred.Competence in Microsoft Office and pharmacy documentation systems is necessary.Must be self-motivated, organized, detail-oriented, and able to work both collaboratively and independently.Preferred Skills :
Experience with provider networks or clinical outreach teams is advantageous.Experience working with the Medicare population is preferred.Knowledge of CMS Star Ratings, HEDIS, and other payer quality programs is beneficial.Ability to analyze pharmacy claims and identify gaps in adherence is a plus.Bilingual skills are an advantage.This position is fully remote, allowing you to work from anywhere within the Tri-State Area of Delaware, New Jersey, or Pennsylvania. You will not need to report to a physical office location to carry out your duties.
