Nurse Practitioner Primary Care

Compensation : Up to $170,000 base salary + Sign-On Bonus

Responsibilities

Provide comprehensive primary care and chronic disease management for adult and senior patients

Coordinate care and support for highly complex dialysis patients (non-nephrology role)

Conduct and document Comprehensive Health Assessments and Health Risk Assessments with a focus on accuracy, holistic care, and specific clinical coding

Administer diagnostic tests and preventive screenings

Develop personalized care plans and educate patients on disease management and wellness

Collaborate with an integrated care team to ensure continuity, quality, and value-based outcomes
