Hello, Sunny Ridge in Nampa, ID is actively looking to hire a full-time MDS Coordinator. We need a detail-oriented Nurse who enjoys working a desk job most of the time, and 1-2 days / week on the floor. We would also accept someone part-time working MDS 3 days a week.

Job Description :
- Responsible to assign ICD-10 CM diagnosis codes to the residents' medical conditions
- Accountable for case management of Medicare Part A and insurance case management
- Expertly assess nursing home residents' needs, and coordinating personalized, resident-driven care based on those assessments
- Oversee the patient assessment process, setting assessment schedules and assuring that assessments are done in an accurate and timely manner
- Make Recommendations and transmit assessments to the Dept. of Health. Review the results to make policy and procedural recommendations to the management staff
- Coordinate patient care plan in accordance with regulatory requirements. Ensure that appropriate facility resources are made available to patients and that patient care is delivered effectively
- Create the schedule for all Medicare and Medicaid including :
- Start Medicare coverage for newly qualified patients and / or send out denial letters
- Keep updated on regulatory changes in Medicare coverage and help determine documents needed for Medicaid reimbursement.

Minimum associate degree.
