As a new MDS Coordinator at Sunny Ridge, you will start your day by reviewing patient assessments and understanding individual care plans. You'll collaborate with nursing staff and healthcare professionals to gather essential data for MDS documentation. Daily responsibilities include conducting resident interviews, ensuring compliance with federal regulations, and maintaining accurate records. You will also participate in interdisciplinary team meetings to discuss patient progress and adjustment of care strategies. Your role will require you to implement quality assurance measures and support ongoing education for staff regarding MDS processes.

Expect to actively monitor safety protocols and advocate for patient needs, all while fostering a collegial atmosphere that thrives on teamwork and open communication.

To excel as an MDS Coordinator at Sunny Ridge, you'll need strong analytical skills to interpret patient data and evaluate care plans critically. Job Description :

  • Responsible to assign ICD-10 CM diagnosis codes to the residents' medical conditions
  • Accountable for case management of Medicare Part A and insurance case management
  • Expertly assess nursing home residents' needs, and coordinating personalized, resident-driven care based on those assessments
  • Oversee the patient assessment process, setting assessment schedules and assuring that assessments are done in an accurate and timely manner
  • Make Recommendations and transmit assessments to the Dept. of Health. Review the results to make policy and procedural recommendations to the management staff
  • Coordinate patient care plan in accordance with regulatory requirements. Ensure that appropriate facility resources are made available to patients and that patient care is delivered effectively
  • Create the schedule for all Medicare and Medicaid including :
  • Start Medicare coverage for newly qualified patients and / or send out denial letters
  • Keep updated on regulatory changes in Medicare coverage and help determine documents needed for Medicaid reimbursement.

Requirements :

  • Minimum associate degree. Bachelor's degree preferred
  • Registered Nurse is preferred.
  • Extensive knowledge in MDS
  • One year of MDS experience in a skilled nursing / long-term care facility
  • Extraordinary attention-to-detail, capable of directing other people and managing schedules
  • Excellent communication skills, expert at working cooperatively as a member of team
  • Proficiency in MDS 3.0
  • Demonstrable working knowledge of relevant state and federal regulations
  • Resident Assessment Coordinator certification
  • Must be able to pass a background check.
