Join our dedicated team as a Home Infusion Clinical Liaison RN, where you will be instrumental in enhancing patient care through effective home infusion therapy services. In this dynamic role, you will build and nurture relationships with discharge planners, healthcare staff, and physicians to facilitate smooth patient transitions to home infusion services. Your primary focus will be on assessing patient eligibility, ensuring appropriate documentation for infusion therapy, and delivering educational support to various stakeholders.

Key Responsibilities :

• Create and maintain strong referral connections with healthcare professionals to boost Home Infusion referrals.
• Collaborate with physicians and facility staff to evaluate the appropriateness of home infusion for patients.
• Provide consultative support including educational presentations about home infusion therapy.
• Act as a clinical resource for ordering providers, staff, patients, and families regarding home infusion therapy.

The base pay range for this role is $46.54 - $72.26, with potential additional compensation available for shift differentials, overtime, and bonuses. Your potential total compensation may vary based on experience and location.
