New England Life Care currently has a Clinical Liaison Nurse opportunity at Baystate Medical Center in Springfield, MA This position typically works Monday - Friday 8AM - 5PM, 1 weekend a month and 1 holiday a year! This position will allow you to work autonomously, have a more predictable schedule, and be able to connect with your patients. Summary :

The Liaison has three primary job responsibilities.

1. S / he provides clinical assessment of patients referred by the member hospitals for home infusion therapy to determine their capabilities to safely receive services at home with NELC.

2. S / he is responsible for developing and implementing strategies to capture all referrals from the member hospital's system of care (e.g. inpatient, ambulatory, etc.).

3. S / he is responsible for relationship building with hospital staff for quality coordination of care between hospital and home
