Job Summary : Provides skilled nursing care to patients in the homes, and responsible for documenting skilled nursing care in accordance with the patients care plan and in accordance with physician’s orders. This is an hourly position.

Qualifications / Educational Requirements :

  • Currently licensed as a Licensed Practical Nurse or Registered Nurse in the state providing care
  • Nursing degree required from accredited / approved professional school of nursing.
  • Minimum of six (6) months nursing experience required, acute care preferred.
  • RB from approved school of professional nursing
  • Current CPR
  • Current PPD, or Chest x-ray if applicable
  • 6 months homecare experience preferred
  • Successful completion of Adaptive’s Competency, and / or evaluation and training program
  • Responsibilities / Essential Functions : The person in this position must be able to perform the following essential job functions with or without reasonable accommodations.

  • Follow the instructions of the nursing supervisor
  • Be able to regularly re-evaluate the needs of patients
  • Completes assessments to determine level of care, if applicable
  • Ability to initiate plan of care and make changes as patient’s level of care changes
  • Establish goals for patients based on diagnosis including input from patient and family
  • Report client / family complaints to agency
  • Administer medications and treatments when necessary, and as prescribed by a physician
  • Maintain client confidentiality / adheres to HIPAA requirements
  • Attend staff meetings, case conferences, team meetings, and educational in-services as required
  • The ability to frequently sit, stand, walk, bend, or squat
  • The ability to occasionally crawl, climb, or reach above shoulder level
  • The ability to occasionally lift up to and over 50 lbs.
  • The ability to continuously see, hear, speak, and use both hands
