Job description
A company is looking for an RN Home Health.
Key Responsibilities
Assess home health and hospice patients to develop care plans based on their needs
Implement and evaluate the Plan of Care to ensure quality service delivery
Educate patients and families on care practices and supervise clinical staff
Required Qualifications
Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program
Bachelor of Science in Nursing preferred
Current state license as a Registered Nurse
Proof of current CPR certification
CHPN certification preferred
