Position Responsibilities :
- Provide compassionate, client-centered services assisting with identifying and accessing more stable housing options.
- Providing trauma-informed, housing-focused case management focusing on identifying safe and appropriate alternatives to emergency shelter.
- Utilize a collaborative process with team members and community partners to provide service linkages and support systems to effectively serve clients.
- Ensure timely and thorough documentation of all services, including in-person meetings and collateral contacts.
- Honor and maintain strict confidentiality in all areas : written, spoken and observed communication.
- Demonstrate knowledge of The Kearney Center and community resources for which people experiencing homelessness qualify and connect clients with these services.
- Demonstrate understanding of the needs of people experiencing homelessness.
- Possess knowledge of programs and policies that impact people experiencing homelessness.
- Participate in weekly supervision and team meetings.
- Assess clients for services with attention to exploring immediately available alternatives to entering the emergency shelter system.
- Determine eligibility for services.
- Coordinate diversion services when applicable, preventing entry into shelter.
- Provide crisis intervention when needed.
- Must demonstrate awareness, acceptance, and ability to work with clients, staff, and community contacts of different cultural and ethnic backgrounds.
- Track eligible client information to provide for entry into the shelter database via HMIS.
- Track, log, summarize and report activities daily.
- Other assigned activities that build the capacities of guests and staff to fulfill the mission of The Kearney Center.
- Some evening hours may be required.
Requirements
Experience with case management and homelessness services preferredBachelor's Degree in Social Work or related human services preferredPay rate is based on skills and experience.
