About The Position
Hardin County we're hiring passionate and self-driven Nurse Practitioners to perform Comprehensive Health Assessments (CHAs) for In-Home and Facility-Based Patients!
Job Details :
- Perform in-home or facility-based CHAs for Medicare, Medicaid, and commercial members
- Conduct noninvasive comprehensive physical examinations for members
- Identify and escalate care gaps and urgent / emergent health concerns
- Complete thorough electronic documentation of the visit using company provided tablet
- Communicate with members' PCP to discuss findings, keeping the care team and leadership informed
- Travel within a 60-mile radius from your assigned location (daily mileage may exceed 120 miles)
Work Environment :

Job Requirements
Requirements :
Board certification (ANCC or AANP)Preferred : Family NP - FNP, Adult Geriatric Primary Care NP - AGNP, Emergency NP - ENP, or Acute Care NP - ACNP
Active KY licenseStrong clinical judgment and ability to work independently in the fieldComfortable with electronic documentation and tablet useWillingness to travel extensively within the assigned 60-mile radiusNew graduates welcome to apply!Benefits
What Our client has to offer :
PRN Role :
Minimum 20 visits per month required - visits may take up to 2 hours; average visit is 1-hour durationVisits scheduled one month in advance based on your availability - whole days preferred, half days accepted90 per completed assessment25 per no-showMileage reimbursementPaid training and provided equipment (tablet, PPE, lab kits, etc.)Must supply personal clinical tools (BP cuffs, scale, stethoscope, etc.)Referral Bonus eligible up to $2000 for qualifying NPs
