Job Requirements

🌟 Now Hiring : RN Care Manager –  York, Maine 🌟

🩺 Position Details :

Start Date : October 31, 2025

Contract Length : 13 weeks

Hours : 40 hours / week

Schedule options : 8 : 30 AM–4 : 30 PM OR four 10-hour shifts (7 : 30 AM–5 : 00 PM)

No on-call required

Weekend & holiday rotation required

💼 Responsibilities :

Conduct patient assessments and coordinate discharge planning

Collaborate with interdisciplinary teams to ensure high-quality care

Manage utilization review and ensure smooth care transitions

Communicate effectively with patients, families, and care partners

🎓 Qualifications :

Active Maine RN license (must be current)

3–5 years of clinical experience (Med / Surg, Case Management, or Home Care)

Utilization review and discharge planning experience required

BLS certification required

CCM or ACM certification preferred
