Qualifications :

MS in Social Work, MSW- Social Services

Minimum of at least one year experience in a medical setting preferred.

Responsibilities :

• Review new admissions and potential discharges
• Perform discharge planning from admission through discharge
• Identify and escalate high-risk long LOS, readmission or complex discharge needs of patients.
• Attend unit-based rounding, department meetings and additional department education / training.
