COMPENSATION : $130,000 BASE TOTAL Potential $143,000+ Full Benefits

Summary :

Optigy is partnering with a leading value-based care organization to hire compassionate and experienced Advanced Practice Providers to deliver high-quality, home-based primary care for a Medicare-aged population in Kansas City. Providers will deliver care directly in patients homes, reducing barriers to access and improving health outcomes for underserved populations.

Key Responsibilities :

Deliver primary care and consultative services in members' homes

Partner with leadership to identify and schedule patients in need of home visits

Collaborate with Medical Assistants and care teams to ensure quality, continuity, and patient satisfaction

Maintain accurate documentation of all services and encounters

Participate in quality improvement activities and ongoing care planning

Provide care within an assigned geographic area with reliable transportation

Qualifications :

Active and unrestricted NP or PA license in Missouri

BLS / ACLS certification

Experience in geriatrics, internal medicine, or family medicine preferred

Comfortable managing patients with multiple chronic conditions

Ability to lift and transport up to 20 lbs. of equipment

Strong communication, empathy, and clinical decision-making skills

Reliable vehicle and valid drivers license required
