California Licensed Clinical Case Manager

A company is looking for an Out of Network (OON) Clinical Case Manager (CM) - Remote (CA Only) - Per diem - Variable.

Key Responsibilities

Coordinate quality healthcare services in out of network facilities and repatriate patients into the healthcare system

Assess, develop, implement, and monitor comprehensive care plans in collaboration with interdisciplinary teams

Facilitate proactive discharge planning and ensure timely communication among healthcare team members

Required Qualifications

3 Years Clinical RN Experience in the ICU, Trauma, or Emergency Room setting

3 Years RN Acute Care, Ambulatory Care, or comparable Case Management experience

California Registered Nurse (RN) license

Driver's License - CA Department of Motor Vehicles
