Registered Nurse Role $2,130 per year About the Role We are looking for a compassionate and experienced Registered Nurse (RN) to join our Program of All-Inclusive Care for the Elderly (PACE) team. The RN will provide direct nursing care, coordinate services, and support participants in maintaining their independence, dignity, and quality of life. Responsibilities Conduct initial and ongoing nursing assessments for PACE participants. Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team. Provide skilled nursing care, including wound care, medication administration, and chronic disease management. Coordinate care across multiple settings (clinic, home, and day center). Educate participants and caregivers on health conditions, treatments, and self-care strategies. Serve as a resource and advocate for participants, ensuring their needs are met. Maintain accurate documentation in compliance with federal, state, and PACE regulations. Participate in interdisciplinary team meetings to promote holistic, person-centered care.
