Physical Therapist (PT)

Location : Hertford, NC

Facility Type : 78-bed skilled nursing & rehabilitation center – short‑term rehab and long‑term care

Schedule : Full-time, flexible shifts

About the Role

As a Physical Therapist, you will be central to restoring mobility, strength, and quality of life for patients recovering from illness, surgery, or trauma. You’ll collaborate with skilled nursing, occupational therapy, physicians, and interdisciplinary teams to deliver tailored, outcomes-driven rehabilitation. Services include orthopedic rehab, sub-acute post-op care, long-term support, stroke / neuro recovery, and more.

What You’ll Do

Design and implement individualized treatment plans for mobility, pain, and function recovery

Lead therapy sessions in an inviting, well-equipped environment

Collaborate with interdisciplinary team members, including nursing, OT, physicians, and therapy aides

Document patient progress and adjust rehab plans for best outcomes

Support patients through sub-acute and long-term recovery pathways

Qualifications

Licensed PT in the state of North Carolina (or eligible for licensure)

Skilled in orthopedic rehabilitation, neurologic recovery, and post-op care

Strong teamwork and communication abilities
