All County Home Health and Hospice - RN Home Health

All County Home Health and Hospice is seeking a talented RN Home Health to join our dynamic team. As an RN Home Health, you will play a crucial role in providing comprehensive home care services to patients and their families. Overview :

The home health registered nurse plans, organizes and directs / coordinates home care services and is experienced in nursing, with emphasis on community health education / experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Duties & Responsibilities :

  • Develop and initiate the plan of care in partnership with the patient, representative, and caregiver(s)
  • Provide preventative and rehabilitative nursing procedures as ordered by the physician
  • Offer patient, caregiver, and family counseling
  • Deliver patient and caregiver education
  • Prepare clinical notes and communicate with the physician and other healthcare practitioners
  • Identify discharge planning needs and implement prior to patient discharge

Communication :

  • Prepare clinical notes and update the primary physician as necessary
  • Communicate with the physician regarding the patient's needs and report any changes in the patient's condition
  • Coordinate care plan with community health-related persons
  • Additional Duties :

  • Participate in the HHA's quality assessment and performance improvement program
  • Participate in on-call duties as defined by the on-call policy
  • Ensure availability of necessary equipment and services
  • Instruct, supervise, and evaluate home health aide care provided every two weeks
  • Supervise Licensed Practical Nurses (LPNs) / Licensed Vocational Nurses (LVNs) and / or Home Health Aids (HHAs) / Certified Nursing Assistants (CNAs), as directed
  • Job Requirements :

  • Graduated from an approved school of nursing and licensed in the state of practice
  • Minimum of two years of experience, with at least one year in public health or home care nursing preferred
  • Current CPR certification
  • Licensed driver with an insured automobile in good working order
  • Self-directed and able to work with minimal supervision
  • Excellent observation, problem-solving, verbal and written communication skills
  • Ability to organize and prioritize workload independently
  • No management experience required, responsible for supervising home health aides
  • Able to perform required nursing skills, including lifting, positioning, and transferring patients
