Job Title : Registered Nurse (RN) - Case Manager

Pay Range - $3000+ / Weekly
Inpatient Case Manager
Schedule : Monday-Friday, 8 AM - 5 PM (some weekend coverage, mostly Saturdays)
Location : Fairfield, CA
Duration : 13-week initial assignment (Possible Extensions)
Key Responsibilities :
- Assess, plan, implement, monitor, and evaluate individualized care plans for hospital inpatients across the continuum of care.
- Collaborate daily with interdisciplinary teams and Social Services to prioritize patient needs and coordinate discharge planning.
- Conduct comprehensive patient assessments and ongoing reassessments to address physical, emotional, and psychosocial needs.
- Utilize clinical knowledge, critical thinking, and problem-solving skills to ensure optimal patient outcomes within financial and health plan resources.
- Communicate effectively with patients, families, physicians, and healthcare staff to support care coordination and transition of care.
- Facilitate referrals and access to community resources such as home health, rehabilitation, long-term care, and hospice services.
- Ensure compliance with regulatory standards, including Medicare, Joint Commission, and Department of Health requirements.
- Maintain accurate documentation in electronic health records (EHR) and case management systems (MCG, MIDAS+, Cerner).
- Participate in quality improvement initiatives and case reviews to promote patient safety and performance excellence.

Skills & Qualifications :
Education : BSN or MSN preferred.Licensure : Current California RN license (required).Certification : Certified Case Manager (CCM) or Accredited Case Manager (ACM) preferred.Experience : Minimum 2 years of acute clinical nursing experience ; case management experience strongly preferred.Technical Skills :
Proficiency in Microsoft Office Suite and healthcare software systems (Cerner, MIDAS+, MCG).Strong data entry, documentation, and computer navigation skills.Knowledge Areas :
Medicare rules, medical necessity guidelines, quality assurance, and outcome determination.Acute, subacute, home, hospice, and rehabilitation care processes.Joint Commission, Patient Safety, and Department of Health standards.Soft Skills :
Strong communication and interpersonal abilities.Excellent organization, prioritization, and time management skills.Team-oriented, collaborative, and adaptable in fast-paced healthcare environments.Demonstrates empathy, integrity, and a commitment to continuous learning.Physical Requirements :
