Qualifications
On an An Average Day you Will :
(Includes, but not limited to)
Complete an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needsProvide a complete physical assessment and history of current and previous illness(es)Provide professional nursing care by utilizing all elements of the nursing processAssess and evaluate patient's status by :Writing and initiating plan of careRegularly re-evaluating patient and family / caregiver needsParticipating in revising the plan of care, as necessaryInitiate the plan of care and make necessary revisions as patient status and needs changeUse health assessment data to determine nursing diagnosisDevelop a care plan that establishes goals, based on nursing diagnosisInclude the patient and the family in the planning processRecord pain / symptom management and changes / outcomes as appropriateCommunicate with the physician regarding the patient's needs and report changes in the patient's condition; obtain / receive physicians' orders, as requiredTeach the patient and family / caregiver self-care techniques, as appropriateWork in concert with the interdisciplinary groupAll other duties as assignedRequirements :
Must have at least one (1) year of acute care experience in an institutional settingOne (1) year of experience in home care preferredMust have a current licensure in the stateMust understand the philosophy of hospice concept and understand needs of the terminally illMust be flexible in work hoursMust demonstrate nursing skills per competency checklistMust demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programsAbility to travel throughout the Bristol Hospice Service AreaWe Got the Perks :
