Nurse Practitioner - Geriatric Primary Care (SNF, LTC, ALF)

Job Description

We are seeking an experienced Nurse Practitioner to provide comprehensive primary care in Skilled Nursing Facilities, Long-Term Care centers, and Assisted Living Facilities.

• Deliver patient assessments, manage acute and chronic conditions, and develop treatment plans.
• Order diagnostics, prescribe medications, and educate patients, families, and staff.
• Collaborate with care teams to ensure seamless, patient-centered care.
• Maintain accurate documentation and support quality improvement initiatives.

Qualifications : Current MN NP license, Board Certification, with 1+ year experience preferred in geriatric or post-acute settings. Strong communication and commitment to patient-centered care are essential.

This full-time role offers a competitive salary ($100K–$140K), flexible scheduling, benefits including health, dental, vision, 401(k), CME allowance, and minimal on-call duties.
