Family Medicine Primary Care Provider

POSITION OBJECTIVE : Responsible for the medical evaluation, treatment and follow-up care of pediatric and adult primary care patients in the medical office, hospital, county prison, long-term care facility settings, and other outreach settings within the scope of practice and geographic are.

Qualifications

Valid license to practice medicine in the states of Vermont and New Hampshire

OTHER : Successful completion of credentialing process for Medical Staff membership and privileges at Cottage Hospital, Woodsville, NH and LRHC; maintenance of active valid narcotic and controlled substances numbers and licenses; Board certification in the specialty of Family Practice.
