The Patient Coordinator is a vital role within our Diabetes RPM program, serving as the primary point of contact for patients. The coordinator supports patients by enrolling them in RPM programs, providing education, monitoring progress, and collaborating with medical teams to ensure exceptional care. This role combines administrative, educational, and support functions to enhance patient engagement and outcomes.
Key Responsibilities
- Patient Enrollment and Engagement
Enroll individuals with diabetes into RPM programs, ensuring they understand program benefits and processes.
Conduct remote consultations to assess patients' health goals and challenges.Educate patients about diabetes management, including nutrition, exercise, and device usage.Patient Monitoring and SupportSchedule and coordinate follow-up appointments with physicians through EMR systems or warm transfers.
Monitor and document patient progress, providing ongoing guidance and support.Offer practical advice and motivation to help patients adhere to their health and wellness plans.Communication and DocumentationMaintain accurate documentation of all patient interactions in electronic health records (EHRs).
Use CRM software and other tools to track patient progress and communicate effectively.Provide efficient and empathetic responses to patient inquiries, ensuring they feel supported and valued.Collaboration and Continuous LearningCollaborate with medical professionals, including endocrinologists and diabetes educators, to deliver holistic care.
Participate in training to stay updated on diabetes care best practices and RPM technologies.Qualifications
Degree / Certificate in Nutrition, Health and Wellness, or a related field preferred.Certified Diabetes Care and Education Specialist (CDCES) certification is a plusPrevious experience in a healthcare call center or patient-facing role is advantageous.Proficiency in MS Office, CRM tools, and EMR systems.Strong organizational and time management skills with the ability to multitask.Exceptional verbal and written communication skills. Fluency in English is required; Spanish proficiency is preferred.A passion for providing excellent patient care and supporting individuals managing chronic conditions like diabetes.Working Conditions
