Case Manager at Adult Day Care Center

Position : Case Manager Location : Bronx NY Type : Full-Time

Job Description : We are seeking a dedicated and compassionate Case Manager to join our team at our Adult Day Care Center. The Case Manager will play a pivotal role in enhancing our existing services by conducting thorough assessments of clients' long-term goals during intake and developing individualized service plans. These plans will incorporate small, achievable tasks aimed at fostering productive daily experiences for our clients.

Principal Responsibilities :

• Manage a caseload of clients.
• Complete Assessment Summaries and determine the type of services needed for each client.
• Develop individualized service plans focusing on recovery and wellness.
• Maintain detailed service documentation, including client progress notes, observations, interactions, and incident reports as necessary.
• Provide crisis intervention support.
• Participate in individual service coordination activities, including services, workshops, and support for recreational and socialization activities.
• Maintain the program's resource directory.
• Refer clients to appropriate community providers to address their needs.
• Perform all other duties as assigned.

Minimum Qualifications :

Bachelor's degree.

At least 1 year of experience working with the elderly and / or adult mental health community.

Ability to develop and implement treatment plans.

Strong problem-solving and crisis intervention skills.

Spanish speaking required.
