General Summary of Duties : As an RN Case Manager, you will provide intermittent nursing services to patients in their homes, collaborating with physicians to create a personalized plan of care that promotes quality of life for terminally ill patients. You will report directly to the Hospice / Palliative Clinical Supervisor and supervise Home Care Aides.

Essential Duties :

Knowledge, Skills, and Requirements : Successful candidates will possess a current RN license in Connecticut and a minimum of one year of recent medical / surgical nursing experience. Knowledge of hospice / palliative care principles, along with strong communication skills and the ability to handle emergency situations, is essential.
