DUTIES :
Provide skilled nursing care, under the direct supervision of the Registered NursePerform selected skilled actions in the provision of curative, rehabilitative, palliative, or preventative nursing care, as necessary, to meet the client's needsPerform direct and indirect care, then report care performed and client observations, through appropriate documentationResponsible, and accountable, for making decisions that are based on the individual's nursing experience and educational preparationAssists in the coordination of patient care, under the direction of the CoordinatorRESPONSIBILITIES
Assist the Registered Nurse in carrying out the plan of careAssist the Registered Nurse in performing specialized procedures, as directed by the Registered Nurse, under physician's ordersAssist with the preparation, implementation, and continuing evaluation of the patient care planRecognize, and understand, the effects of social and economic problems upon patients and provide for the emotional and physical comfort and safety of patientsHelp teach the patient the appropriate self-care techniquesFoster cooperative effort among personnel, by understanding the functions of other persons involved in patient care, and by active participation in team and staff conferencesAssume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on the job learningPrepare, and submit, clinical and progress notes, no less often than, weekly (not to include admission)Observe, record, and report, to the appropriate person, patient symptoms, reactions, and changes including :The general physical and mental condition of patients and signs and symptoms, which may be indicative of changes in conditionStresses in human relationships, between the patients and staff personnel, and between patients and their families and visitorsAssist with the rehabilitation of patients, according to the medical care plan, by encouraging the interests and special aptitudes of the patient and encouraging patients to help themselves using their own capabilitiesFurther assist in rehabilitation by reporting any abnormality in the patient's range of motion, body mechanics, or body alignment.Actively participate in nursing organizations, promote, and participate in in-service programsRequirements
ENVIRONMENT AND PHYSICAL REQUIREMENTS
Contact with clients under a wide variety of circumstances.Care is provided in client's living environment with varying situations, including private homes, independent or assisted living or in an institution such as a hospital or long-term care facility.May be required to respond to emergency situations.Travel required.Position includes tasks that have the potential for exposure to blood / body fluids / tissues and other potentially infectious material such as body fluids / tissues.PHYSICAL REQUIREMENTS :
Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull.Must be able to lift up to 50 lbs.Corrected visual and hearing acuity required.QUALIFICATIONS :
1. Graduation from an accredited School of Practical Nursing
2. Must be licensed, in the state as a Licensed Practical Nurse
3. One, or more years, of experience, in community / Home Health Agency setting, is preferred
4. Must have a criminal background check
5. Must have current CPR certification. Online certification is not accepted.
6. The ability to communicate well, both, verbally, and in writing, in English, is required.

Pay : $29.93 - $31.72 per hour
