Licensed Practical Nurse (LPN)

Join our team at Berlin Rehabilitation & Healthcare Center as a Licensed Practical Nurse (LPN)!

Full Time 7A-7P and Part Time 7A-3P

PER-DIEM : 7A-3P, 3P-11P, 7A-7P, 7P-7A SHIFTS AVAILABLE!

(One weekend per month, one winter Holiday and one summer Holiday is required)

Pay Range : $30.00 / hr. - $39.00 / hr. (All Inclusive)

Responsibilities of LPN :

Provide patient care in accordance with acceptable standards of nursing practice and the written plan of care, as well as physician’s plan of treatment

Follow a plan of care designed by Nurse Manager

Perform medication administration and treatments per plan of care (POC)

Monitor residents’ conditions and report accordingly

Maintain accurate documentation in accordance with orders provided

Qualifications of LPN :

A current, unencumbered active license to practice as an LPN in the state of New Jersey.

Must have valid CPR certification (BLS)

Ability to work with multiple patients at once

Minimum 2+ years nursing experience in a hospital, nursing home or other healthcare setting - preferred

Effective communication skills, professionalism, and ability to harmoniously communicate with personnel, residents, family members, visitors, government agencies / personnel, and the public.

Strong knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care centers.

Experience with Point Click Care (PCC)- preferred
