Job Overview

The Linkage to Care Coordinator plays a vital role in facilitating the connection between individuals in need of healthcare services and the appropriate care providers. They are responsible for guiding and supporting patients, ensuring seamless transitions to care, and promoting health equity and access to healthcare resources. The coordinator works in collaboration with healthcare teams, community organizations, and patients to ensure the successful linkage to necessary services.

Minimum Requirements

1. Bachelor's degree in healthcare, social work, or a related field.

2. Previous experience in healthcare coordination, case management, or social services preferred.

3. Knowledge of healthcare disparities and social determinants of health is an asset.
