COMPENSATION : Making $285,000 + Signing Bonus up to $50k

Transportation is provided to patients that need it at no charge.

Role :

• Compensation : Base $240-$260k based on experience
• Bonus : Up to 10$ of base salary
• Partnership : No Buy-in.
• 18-22 patients a day
• Hours : Monday Friday work schedule; Clinic Hours 7 : 30a 5 : 00p (last patient is scheduled at 4 : 15)
• Work / Life Balance
• Small Patient Panel 450 patients max
• Administrative time included in schedule
• No nights or in hospital call
• 100% outpatient
• Dedicated Care and support team (have your own MA / Scheduler / Referral specialist)
• Patient Population : Seniors with 3-5 chronic diseases
• Value Based Care
