RN Care Coordinator

Hourly Rate : $52.65 - $60.00 starting, DOE. Schedule : 4 days a week, 8 hour shift of 8am-5pm Benefit Highlights (Pro-rated for Part Time) : Employer-paid Health Insurance (medical, dental and vision) : 100% for employee and 80% for dependents. 17 days PTO per year, 11 Paid Holidays, 8+ Sick Days (1 hour for every 30 hours worked) Retirement Plan with 6% match Job Duties : Deliver individualized and group diabetes care Coordinate outcomes, education and community outreach Collaborate with providers and other staff for holistic care Minimum Qualification : Current RN License with 2 years experience. Current BLS certification.
