Job Purpose
The Per-Diem Speech Language Pathologist is responsible for implementing standards of care in speech-language pathology services, ensuring high-quality patient outcomes in the home health and hospice setting.
Primary Duties and Responsibilities
Conduct comprehensive evaluations and assessments of the patient's home environment, identifying equipment needs related to speech and language functions.Identify functional speech defects and develop a plan of care aimed at improving the patient's speech and language abilities.Administer speech therapy programs using specialized therapeutic techniques and / or equipment.Recommend and facilitate the use of communication devices or aids as needed.Collaborate with referring physicians and other healthcare team members to ensure coordinated, comprehensive care as part of the patient's total plan of care.Participate in case conferences, in-service training, and other meetings as required to maintain coordinated and comprehensive care.Prepare and submit accurate and timely clinical notes in accordance with organization policy.Maintain clinical competency in speech-language pathology practice and theory.Identify additional patient and family / caregiver needs and make appropriate referrals to other services.Instruct patients, family members, caregivers, and other healthcare personnel in the patient's treatment plan as needed.Supervise home health aides in procedures directly related to the speech-language therapy plan of care when applicable.Perform other duties as required to facilitate the delivery of speech-language pathology services.Note : The above statements are intended to represent the major duties and responsibilities of this role. Qualifications
Master's or doctoral degree in Speech-Language Pathology (SLP).Licensed as a Speech-Language Pathologist by the State or meets the requirements for unlicensed states, including :Completion of 350 clock hours of supervised clinical practicum.
A minimum of 9 months of supervised full-time SLP services after obtaining the degree.Successful completion of an approved national examination in SLP.At least one (1) year of clinical experience in Speech-Language Pathology.Strong verbal and written communication, and organizational skills.Current CPR certification.Licensed driver with a reliable vehicle, insured in accordance with organization requirements.Working Conditions
This is a traveling position requiring a reliable vehicle to navigate varied weather conditions within the geographic area.Physical Requirements
Prolonged walking or standing.Ability to lift, position, or transfer patients and lift supplies and equipment.Considerable reaching, stooping, bending, kneeling, or crouching.Visual acuity and hearing necessary to perform required skills.Benefits :
