Summary :

Supervises the daily staff activities of Social Worker staff in addition to their normal social worker responsibilities. The Social Worker Supervisor provides patient education and emotional support, is responsible for referrals to community agencies, coordinates patient care with other team members, and assists with concrete services.

Responsibilities :

Responsible for program's patients' social needs including discharge planning for inpatient and outpatient. Responsible for evaluations to include psychosocial assessments in order to determine appropriate candidacy and continues to work with patient and family. Provides ongoing assistance with social needs to all patients. Responsible for the presentation of patients at selection committee as required by CMS, where appropriate.Performs staff responsibilities to fulfill required service levels. Leads the handling and resolution of complex issues and complaints.Assesses quality of services delivered and facilitates staff development programs. Ensures staff compliance with departmental and organizational policies, procedures, and protocols.Provides direction, orientation, training, coaching, and mentoring to staff. Performs or assists with performance evaluations and disciplinary actions.Oversees and is a resource to specialty social workers.

Masters : Social Work (Required)Fingerprint Screen (FINGERPRNT) - Florida Department of Childrens & Family (DCF)

Additional Job Information :

Complexity of Work : Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Required Work Experience : Five (5) years' experience. Other Information : Additional Information : Employment in this position is contingent upon successful completion of a Level II Criminal History record check.Additional Certification Info : Certified Clinical Transplant Social Worker (CCTSW) credential preferred, where applicable. LCSW preferred.Active participation in social work professional organizations preferred.
