Position Summary
The Physical Therapist (PT) plays a vital role in delivering direct rehab care to patients referred by physicians. You will work in accordance with all applicable laws, regulations, and Life Care standards, ensuring the highest quality of care.
Education, Experience, and Licensure Requirements
- Degree : Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT). If graduated in 2003 or later, must have MSPT or DPT.
- Licensure : Currently registered / licensed in the applicable state, maintaining an active license in good standing throughout your employment.
- Experience : One (1) year of experience in post-acute care or a related setting is preferred.
- CPR Certification : Must be obtained upon hire or during orientation, and must remain current throughout employment.
Specific Job Requirements
Engage in community health initiatives and projects as appropriate.Proficient in Microsoft Word, Excel, and email communication.Collaborate with patients, families, and support departments to effectively plan for patient needs.Demonstrate excellent body mechanics at all times.Make independent decisions when necessary.Stay informed about practices, procedures, and regulations governing post-acute care facilities.Implement and interpret departmental programs, goals, and policies.Maintain professional relationships with associates and vendors.Uphold confidentiality of all proprietary information.Adhere to company policies, including harassment and compliance procedures.Exhibit integrity and professionalism by following Life Care’s Code of Conduct .Essential Functions
Follow and implement physical therapy treatment plans for patients.Establish, assess, and modify realistic, measurable, and functional goals.Oversee and evaluate care provided by PTAs, Rehab Aides, and students.Document patient care appropriately and in a timely manner.Utilize therapy software accurately and effectively.Provide excellent customer service and maintain a positive attitude towards patients.Assist in the safe evacuation of patients when necessary.Demonstrate dependable attendance and punctuality.Utilize reasoning skills and good judgment in all situations.Communicate effectively and work productively within an interdisciplinary team.Engage in physical activities such as sitting, standing, bending, lifting, and walking throughout your working hours.Read, write, speak, and understand the English language proficiently.Join us and be part of a team that values your skills and dedication!
