Job Description for Licensed Practical Nurses (LPNs)

Summary :

The LPN, under the guidance of the RN, seeks to provide the most appropriate health care monitoring and nursing interventions with expected outcomes directly affecting each Individual's unique capacities and ability to participate in the daily program.

Essential Job Duties and Responsibilities :

  • Provide observation, monitoring, and documentation for all Individuals we serve on presenting health needs.
  • Administer medications as necessary.
  • Perform medical rounds, nursing procedures, and other treatments as required.
  • Maintain accurate, neat, concise, and timely documentation in all medical and agency records.
  • Participate in interdisciplinary and other health-related meetings as requested.
  • Assist the Registered Nurse in addressing health teaching needs of the Individuals we serve, their families, and staff.
  • Attend continuing education seminars on appropriate topics as able.
  • Mandatory attendance at required in-service training, along with the ability to absorb and demonstrate compliance with all regulations and RCM Health Care Services policy and procedure requirements.
  • Learn and master the use of any and all software systems and technology utilized by the Agency, on a Program by Program basis.
  • Attendance / Punctuality : Consistently at work and on time. Ensures work responsibilities are covered when absent. Participates in team meetings and case reviews as assigned.
  • Review visit summaries and follow up on recommendations.
  • Ensure the MARs are accurate and that AMAPs are administering medications as prescribed.
  • Conduct member assessments.
  • Provide in-services as needed.
  • Address any additional needs to support the members' care.

Supervisory Responsibilities :

  • Assist RN in supervising staff in carrying out medical care, up to and including supervising some employees in administering medication.

  • Physical Demands :

    While performing the duties of this job, the employee is required to stand, sit, push, pull, lift and / or transfer individuals, use hands to finger, handle or feel, talk and hear. Specific vision abilities include close vision, distance vision, and the ability to adjust focus.

  • Qualifications :

  • Must be licensed to practice as an LPN in New York State.
  • Must be able to travel to different locations when needed.
