JOB SUMMARY

Provides home health services consistent with the mission, vision and goals of the Agency. Physical Therapy Assistant is a member of the home health team who assists in the provision of physical therapy and the coordination of patient care under the supervision of a Physical Therapist to provide treatment of disabilities, diseases, or other conditions of health by the use of physical, chemical, and other properties of air, cold, heat, electricity, exercise, massage, ultrasound, water or equipment in accordance with a written plan of treatment prescribed by a physician.

DUTIES AND RESPONSIBILITIES

PHYSICAL DEMANDS AND WORKING ENVIRONMENT

Must have visual and hearing acuity. Communicates verbally in a clear fashion.. Must be able to read, write and speak English fluently. May be exposed to minor hazards / communicable diseases. Must provide proof of auto insurance upon hire and each renewal and maintain a safe driving record.
