Position : Home Care Hospice Case Manager

Location : Colorado Springs, CO

Duration : 12 weeks

Hours : Monday through Friday, 8 AM - 5 PM (No weekends)

Pay Rate : Up to $57 per hour

Job Description : Are you an experienced Hospice Case Manager looking for a rewarding contract position? We are seeking a compassionate and skilled Home Care Hospice Case Manager for a 12-week contract in Colorado Springs, CO. Enjoy a Monday through Friday schedule with no weekends and earn up to $57 per hour while making a significant impact on patients' lives.

Responsibilities :

• Manage and coordinate hospice care for patients in their homes.
• Develop and implement individualized care plans.
• Provide compassionate support and education to patients and their families.
• Collaborate with interdisciplinary team members to ensure comprehensive care.
• Document patient progress and adjust care plans as needed.
• Travel to patients' homes using reliable transportation.
