Care Navigator

A company is looking for a Care Navigator to manage and coordinate patient referrals to external medical specialists. Key Responsibilities Process referrals to secondary care and maintain directories of specialists and community services Ensure timely communication and documentation of referrals to maintain continuity of care Act as the main point of contact between patients, specialists, and clinic physicians regarding external care needs Required Qualifications At least 3 years of applicable experience, preferably in a Primary Care setting High school diploma or GED required Proficient in Google spreadsheets and / or Excel, as well as database management
