is seeking a dedicated and compassionate

Advanced Practice Provider (Nurse Practitioner or Physician Assistant)

to join our

Primary Care

team. This full-time role offers the opportunity to deliver comprehensive, patient-centered care in an established outpatient family medicine setting. Ideal candidates are clinically skilled, community-focused, and committed to delivering high-quality care to patients of all ages.

Position Summary & Qualifications

Full-time position in a busy outpatient

Primary Care

clinic

Provide full-spectrum care including preventive health, chronic disease management, and acute care visits

Collaborate with physicians, other APPs, and clinical staff for coordinated, team-based care

Maintain timely documentation in the electronic medical record (EMR) system

Schedule : Monday-Friday, with minimal call responsibilities

Must be a

licensed NP or PA

in the state of Illinois

Board certified in

Family Medicine (NP)

or possess the appropriate PA certification

Minimum

12 years primary care experience

preferred

Strong clinical decision-making, interpersonal, and documentation skills

Bilingual candidates are encouraged to apply

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $110,000.00 to $125,000.00 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
