About the Role :

As a Licensed Vocational Nurse (LVN) at Mission Community Hospital, you will play a crucial role in providing high-quality patient care under the supervision of registered nurses and physicians. Your primary responsibility will be to assist in the assessment, planning, implementation, and evaluation of patient care plans. You will be instrumental in monitoring patients' vital signs, administering medications, and providing basic bedside care, ensuring that each patient receives compassionate and effective treatment. Additionally, you will collaborate with healthcare teams to communicate patient progress and any changes in condition, contributing to a holistic approach to health care. Minimum Qualifications :

  • Current and valid Licensed Vocational Nurse (LVN) license in the state of practice.
  • Completion of an accredited vocational nursing program.
  • Must have SNF or LTACH experience

Responsibilities :

  • Provide direct patient care, including monitoring vital signs and administering medications as prescribed.
  • Assist in the development and implementation of individualized care plans for patients.
  • Communicate effectively with patients, families, and healthcare team members regarding patient status and care needs.
  • Document patient care activities accurately and timely in electronic health records.
  • Participate in patient education and support, helping patients understand their health conditions and treatment options.
  • Skills :
