Your Role as an RN :
Conduct patient visits as assigned, performing assessments, eligibility determinations, admissions processing, care planning, implementation, and evaluations.Gather essential data to assess the physical, psychological, social, and spiritual needs of patients and their families, coordinating with the interdisciplinary team.Communicate with physicians and other care team members to ensure optimal care delivery.Regularly evaluate and update care plans based on patient needs.Execute therapeutic, preventable, and rehabilitative nursing procedures as prescribed.Provide clinical pronouncements of death and emotional support to families, when applicable to licensure.Maintain accurate and timely documentation of patient records and care plans.Qualifications :
Must be a Registered Nurse, having graduated from an accredited nursing program and hold a valid state license.At least one year of clinical experience, ideally in intensive care, rehabilitation, or medical-surgical nursing.Home health experience is a plus!Valid CPR certification is required.Must have reliable transportation, a current driver's license, and valid automobile insurance.Join us in providing quality care and making a difference in your community!
