As a Home Health Registered Nurse (RN) Case Manager, here’s a big picture view of what you’ll do :
Oversee the delivery of clinical and caregiver services, ensuring the highest quality of patient / client care in accordance with home health laws and regulatory standardsOASIS admissions and Coordinate with a team of LPNs, CNAs, aides and to ensure client and patient goals are metFollow the Home Health orders from PCPs working closely with their officeEnsure medical supplies / equipment are delivered and staff are trained on care proceduresAssess patient / client needs, resolve issues, and ensure a high level of satisfactionEffectively manage financial resources related to the delivery of quality patient / client careA few must-haves for a Home Health Registered Nurse (RN) Case Manager :
Graduate of an accredited nursing program and active RN or BSN license in MassachusettsMinimum of 1 year of nursing experience in home healthcare / OASIS admissions, recerts, resumptions and DC’sAt least 1 year of supervisory and caregiver management experienceCPR certificationExtensive knowledge of state and federal home health laws and regulatory standardsExcellent management, organizational, problem-solving, communication and interpersonal skillsactivities that may be required.
