Physical Therapist - Days - 13-15 patients / day, 10 hours days, time split between 2 clinis within 1 hour drive between the two, 80 / 20 inpt / outpt. RTO : No more than 3 days may be requested per 13-week contract. Facility may cancel up to 3 shifts per 13-week assignment and guaranteed hours will not apply. Flu vaccine is required NO exceptions. Requirements : active PT license in the state of AZ, BLS, pass the NPTE
