Title : RN-Case Management

Location : San Diego, CA

Pay : $78- $80 / hr

Shift : 8h day (variable start times : 0800-0930).

Key Job Responsibilities :

1. Provides care to patient and family care that progresses them towards self-identified, holistic, and individualized goals. 2. Performs nursing process through individualized assessment, planning, implementation and evaluation of safe, therapeutic, efficient care for patients.

3. Promotes quality outcomes.

4. Collaborates or seeks additional resources as needed in the management of complex, new / unusual, or other high-risk clinical situations

5. Serves as patient and family advocate in ensuring excellent and satisfying care.

Sharp HealthCare Registry RN Discipline - Specific Responsibilities :

Provides safe, age-appropriate, individualized nursing care using the nursing process.

Assesses patients and families, identifies priority needs, and documents findings.

Develops, implements, and evaluates patient-centered plans of care (IPOC / IPER) with the healthcare team.

Adapts nursing interventions based on patient condition, readiness to learn, and cultural needs.

Educates patients / families, evaluates learning, and adjusts teaching plans as needed.

Acts as a patient advocate, promoting participation in care decisions and protecting patient rights.

Communicates effectively with patients, families, and the interdisciplinary team to coordinate care.

Delegates tasks appropriately within scope and competency of team members.

Participates in care conferences and ensures follow-through on interventions, referrals, and workflow coordination.

Licensure :

CA RN license

Certifications :

CPR

Case Manager cert (ACM or CCM) is preferred

Required :

At least three years of recent RN experience working in RN role.

At least two years of recent RN experience working in a specialty area.

Must live 50 miles away from the facility

Travel experience mandatory
