Job Title : LPN / LVN - Outpatient Rehab & Mental Health

Location : Ithaca, NY

Duration : 13 Weeks | 8-hour shifts, Monday-Friday

Pay : $25-$35 / hour

Position Summary :

"Navitas Healthcare, LLC" is seeking LPN / LVN - Outpatient Rehab & Mental Health for an exciting job in Ithaca, NY.

Key Responsibilities :

• Prepare, calibrate, and administer medications, including liquid methadone and occasional injectables, starting at 7 : 00 AM.
• Conduct wellness checks, including vitals, blood pressure, and general assessments.
• Assist Nurse Practitioners with rooming patients and documentation.
• Collect and process urine samples; verify patient information.
• Maintain strict accuracy with controlled substances; medication errors are not permissible.
• Document care in Epic and manage dosing using Evolve software.
• Ensure compliance with facility protocols, safety standards, and patient confidentiality.
• Float between Ithaca and Cortland locations as scheduled.
